This explains standard policy and asks for your signed consent to provide optional contact lens (re) fitting, (re) evaluation and necessary follow-ups for your contact lens prescription and care.

This explains how 'vision' plans (compared to 'medical' plans) help pay for eyeglasses and contact lenses.  'Medical' plans (Aenta, United, Blue Cross, Cigna, etc.) typically do not provide coverage for eyeglasses or contact lenses.  However, 'vision' plans, like EyeMed, Vision Service Plan and Comp Benefits will help pay for a portion of these things.

If you are using Medicare to help pay for your services, this form must be completed.

This form tells you how our office will use your eye care information.  It also allows you to set limitations on how communication is forwarded regarding your care.
